Travel History

Please complete the following form upon your return from work if you travel outside Teton County.

As a reminder:

• When traveling we ask you to use your judgement and distance as much as possible, use hand hygiene and masking if you have to be in large groups
• Upon return we ask that you mask and symptom monitor, which is already required if you are working, and notify employee health or your provider if any symptoms appear (fever, cough, sore throat, loss of smell or taste, etc.)
• If you choose to travel to a “hot spot” that results in you being quarantined, you will not qualify for LTI. You can use PTO or apply for unemployment. If your position allows you to work from home while quarantined, you may continue to work
