2018 Community Health Needs Assessment Report Available

The 2018 Community Health Needs Assessment Report is now available online at http://healthytetoncounty.org. Healthy Teton County noted the community’s top community health issues to be mental health, alcohol, sexual and reproductive health, chronic disease and cancer screenings, nicotine use, and immunizations for those 65+. The prioritized social determinants of health were severe housing, access to care, and food insecurity. The 2018 Community Health Implementation Plan, which outlines the goals and strategies for addressing each identified issue, will be available later this summer.

This is the second iteration of the Community Health Needs Assessment conducted by Healthy Teton County, a community health initiative led by Teton County Health Department and St. John’s Medical Center, with participation from over 35 local organizations.
