How to Access Your Medical Records at St. John’s Health

If you would like to request copies of your medical records and/or a copy of your radiology images, please download and complete this authorization form (Adobe PDF format).

You can return the form by:

The authorization form needs to be completely filled out before medical records can be released. Please note, incomplete forms cannot be processed. We cannot accept electronic signatures at this time. You will need to print this form and sign it. It can be scanned back into an email and returned by email, fax or mail.

If you would like assistance in completing the form, you can come to St. John's Health Information Management Department and we would be happy to assist you, or you can call us at 307.739.7490.

Please allow for 5 to 7 business days to process your medical record request.

Your medical records (including radiology images) may be emailed, faxed, mailed, or picked up. Please mark the appropriate box on the authorization form to indicate how you would like to receive your records. If you would still like a CD please write CD next to the radiology images box. Please note that you cannot open the CD on a MAC or with Windows 10

Your health records are also available online (with some exceptions, like radiology images and some sensitive test results).

Don't have a portal account yet?

Set up an account: While registering for your appointment at St. John’s Health or St. John’s Physician Practices, you will be asked to provide your personal email address and select a security challenge question and answer. Then an invitation with a secure link will be emailed to you to complete your enrollment.

For follow-up appointments, an authorization is not needed. You or your provider may call 307.739.7490 and request to have your medical records faxed directly to the provider.
