What are you looking for in your health care? At St. John’s Health, we value patient-centered service, clinical excellence, and community wellness. We handpick our staff and affiliated physicians for their medical expertise and compassion for patients, so that you have a positive experience with our team. When you visit our center as either a patient or a visitor, you can expect to be treated with respect and greeted by a staff that will spare no effort to help.

Accessibility

St. John’s is committed to providing access for all patients and visitors with disabilities. The main entrances to the hospital are wheelchair accessible, and accessible restrooms are available in many areas.

Case Management/Discharge Planning

If you are an inpatient at St. John’s, your physician or our social worker will help develop a discharge plan to prepare you and your family for your return home..

When you’re discharged from the hospital, our staff works to make sure you understand your follow-up treatment needs. That includes knowing your medications, making appointments, and resuming regular activities. Speak up if you have any questions, we are here to make sure your recovery continues even after you leave the hospital.

Advance Healthcare Directive

You have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. The form below lets you do either or both of these things. It also lets you express your wishes regarding donation of organs and the designation of your supervising health care provider.

If you use this form, you may complete or modify all or any part of it. You are free to use a different form, but you should make sure it is effective under Wyoming law before using it.
