The Geriatric Connections Assessment is a comprehensive evaluation designed to get a clear picture of an older adult’s physical, cognitive, and emotional health and to build a plan that supports their safety, independence, and wellbeing.
The assessment is conducted by a team of providers from St. John’s Health, including a physician or nurse practitioner, a speech-language pathologist, an occupational therapist, and a social worker. Together, they evaluate:
