Complete an Advance Directive at the Health Fair

In addition to getting free medical screenings at the Community Health Fair, attendees can also cross another item off their list: completing an advance health care directive. Printed copies will be available at the Health Fair, and staff members will be on hand to assist with any questions. The Health Fair takes place Saturday, May 4, 9 am–noon, at the Teton County/Jackson Rec Center at 155 E. Gill Avenue.

An advance directive is a legal document in which a person specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or injury. An advance directive become part of your health care record.

An advance directive includes power of attorney for health care, giving specific instructions about any aspect of your health care, donate your bodily organs and tissues following your death, and designating a supervising health care provider to have primary responsibility for your health care.
