Home & Hospice Care in Jackson Hole

Personalized Care for You & Your Loved Ones

Realizing a loved one needs home care is never easy. Many people feel guilty at the thought of entrusting their loved one's full-time care to someone else, but oftentimes this is the best thing for them. When a condition has advanced to the point where home or hospice care is required, your loved one will benefit from having a trained medical professional nearby. What’s most important in this situation is finding a caretaker you can trust to give your loved one the compassionate, thorough service they need.

At St. John’s Health, patients and their families are part of the team. We create personalized care programs so you can rest assured your loved one is receiving the care you know they deserve.

Home Care

Having a home caretaker is an excellent way to give your loved one valuable medical attention and improve their quality of life. Home care is an excellent service that helps the patient retain a sense of independence and peace of mind. Our home health workers have been trained in a variety of medical specialties to aid patients afflicted with various conditions.

Home health care can be beneficial for:

  • Heart disease
  • Respiratory illnesses
  • Cancer
  • Orthopedic injuries
  • Surgery recovery
  • The elderly

We accept Medicare, Medicaid, and most commercial health insurance. Our multidisciplinary team of Home-Based Services professionals provides care to our clients with a focus on total family health. We can assess and treat you at home under the direction of your physician.

Personal Emergency Response System

To help patients maintain their independence, we have created a personal emergency response program. Elderly or ill patients at risk of suffering sudden, unexpected issues can wear a simple device around their neck that, when pushed, alerts 24 hour on-call medical professionals. Financial assistance for this system is available through the Mary Mead Emergency Response Fund.

St. John’s Hospice

St. John’s Hospice does not just provide patients with a place to receive treatment, we strive to improve their quality of life, provide their family with support, and make the transition from home to hospice as smooth and convenient as possible. St. John’s hospice provides compassionate end of life care from specially trained medical specialists. During this difficult time, our team acts as your partner as you deal with the emotional impact of this decision, either as a patient of a loved one.

Both home and hospice care include the following services:

  • Skilled nursing care
  • IV therapy
  • Nutritional counseling
  • Support services (bathing, dressing, etc.)
  • Social work, including counseling on financial issues
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Health screenings
  • Support groups

St. John’s Hospice Care takes an integrated approach, which involves the hospice medical director, skilled nurses, social workers, pharmacists, certified nursing assistants, spiritual care, trained volunteers, grief and bereavement support, and, of course, the patient and their families.

Hospice Volunteer Opportunities

We are always looking for compassionate volunteers to aid our hospice team. These generous individuals help our patients by spending time with patients to provide them with emotional support and a reassuring presence. Simply reading a book or listening to music with a patient can make a tremendous difference. High school and college students in need of community service hours may find work here that is both emotionally fulfilling and easy to work with their schedule.
