Geriatric Connections Assessment

Supporting Independence and Quality of Life for Older Adults in Jackson Hole

Are you noticing changes in a loved one’s memory, balance, mood, or ability to manage daily life? These shifts can be subtle and hard to evaluate on your own. The Geriatric Connections Assessment is a comprehensive evaluation designed to get a clear picture of an older adult’s physical, cognitive, and emotional health and to build a plan that supports their safety, independence, and wellbeing.

How It Works

The assessment is conducted by a team of providers from St. John’s Health, including a physician or nurse practitioner, a speech-language pathologist, an occupational therapist, and a social worker. Together, they evaluate:

  • Memory, thinking, and judgment
  • Balance, mobility, and fall risk
  • Mood and emotional wellbeing
  • Daily functioning and self-care
  • Medication management
  • Nutrition and weight changes
  • Vision, hearing, and continence
  • Living situation and social support
  • Goals of care and advance care preferences

After the assessment:

After the evaluation, the team works together to design a personalized plan of care, one that’s tailored to your loved one’s specific needs and focused on helping them live as safely and independently as possible.

Who It’s For

This program is for older adults and their families who want a thorough, unhurried look at how aging is affecting daily life. It’s especially helpful when you’re noticing changes but aren’t sure what they mean or where to start.

This program is comprised of a team of health care providers from St. John's Health and Teton Physical Therapy. During your evaluation you’ll meet with:

  • A speech language pathologist to assess thinking, memory, and judgment
  • An occupational therapist for assessing balance, sensation in hands and feet.
  • A social worker to offer psychological, social, and emotional support.
  • A physician to discuss general health, wellbeing, and any medical concerns you may have.
